Chapter 6: The Second Wave

Before the family could fully catch their breath, Elena’s body stiffened violently once more, a fresh wave of contractions tearing through her abdomen with terrifying speed. The joy of the first birth was instantly eclipsed by a sharp spike of renewed panic as Martha noticed the sudden change in Elena’s posture and the alarming pallor spreading across her cheeks.
"Martha, it's starting again," Elena whimpered, her fingers clawing weakly at Caleb’s forearm. "There's... there's another one. I can feel it."
"Of course there is, we knew twins were coming, but not this fast," Martha said, her voice retaining a veneer of calm that masked her internal racing thoughts. The first delivery had drained a significant amount of Elena’s energy, and the sudden drop in blood pressure was a dangerous sign. "Caleb, drop the towels and listen to me. The phone. Pick it up off the floor. Tell the dispatcher the first baby is out, but the second one is arriving right now. Tell them we need the paramedics inside the house immediately, no excuses about the gate."
Caleb snatched the smartphone off the wooden floor, his voice cracking as he shouted into the speaker. "We have one! A girl! But the second one is coming right now! Get inside this house! Break the front door down if you have to!"
"Sir, stay calm, units are turning onto your street right now," the dispatcher's voice replied, though it was nearly drowned out by Elena’s renewed scream.
Martha positioned herself again, her experienced hands assessing the position of the second infant. Her heart sank slightly; the baby was breech, presenting feet-first in a hasty descent that left no margin for error. In a sterile hospital operating room with a full surgical team, this would require a swift cesarean section. Here, on the floor of a suburban hallway with fading sunlight casting long shadows through the front door, they were entirely on their own.
"Elena, you have to listen to me very carefully," Martha said, looking straight into Elena’s dilated, terrified eyes. "This little one is coming feet-first. We don't have time to wait for the doctors. You are going to have to push with everything you have left inside you. Can you do that for me?"

Elena closed her eyes, tears streaming into her hair. "I’m so tired, Martha. I don't have anything left."
"You have love, and you have courage," Martha urged, squeezing Elena’s shoulder with fierce intensity. "And your little girl in your arms is waiting to meet her twin. Push, Elena. Push!"
